*Follow the interdisciplinary plan of care, be alert to the patient's needs and concerns

*Provide personal care, including bathing, dressing, grooming, hair, nail, skin and foot care and oral hygiene

*Assist patients, according to the plan of care in: Turning, Sitting, Standing.

*Ambulation with elimination, including: facilitation of bedpans, and urinals. Assisting patient to the bathroom. Routine Foley care-Enemas, Meals, when necessary.

*Perform simple procedures as an extension of therapeutic services, in accordance with the interdisciplinary plan of care

*Observe and report changes in the patient's condition and needs to the Team Director (TD) or Registered Nurse (RN)

*May perform simple homemaking chores for the patient directly, meal preparation, and/or light housekeeping in the patient's immediate area

*Complete required documentation per policy

*Participate in continuing education programs

*Follow confidentiality regarding patients and families per policy

*Demonstrate a compassionate attitude toward patients and families consistent with the philosophy and mission of the organization

*Attend patient care conferences as requested by the TD

*Perform other related duties and special projects, as assigned

*Perform other duties as assigned including those within the scope of current, valid professional licensure/credentials.
