Director of Case Management

She Recruits is seeking a highly experienced Director of Case Management for a large acute-care hospital. This leader will oversee care coordination, discharge planning, utilization review, and social work functions while ensuring high-quality patient outcomes and operational excellence. Candidates must bring strong acute hospital case management leadership experience and be comfortable directing a complex, high-volume environment.

The Director of Case Management provides strategic direction, operational oversight, and performance leadership for all case management and social work services. This includes evaluating departmental processes, optimizing staffing and resources, ensuring compliance with regulatory and organizational policies, and partnering closely with clinical and administrative leadership to advance patient throughput and quality outcomes.

Key responsibilities include leading and evaluating all case management operations, managing FTE allocation, ensuring proper execution of care coordination and discharge planning workflows, overseeing core case management functions, partnering with the CFO and Division CM Leader to achieve financial and operational targets, using data analysis and process improvement strategies to drive sustained performance improvements, ensuring compliance with policies and regulatory requirements, building strong relationships with CMO, CNO, ancillary leaders, compliance, and legal teams, providing leadership, coaching, and accountability for both direct and indirect reports, and upholding organizational mission, values, and code of conduct.

Required qualifications include 3+ years of recent acute hospital case management experience, 2+ years of case management leadership, a Bachelor's degree required, a Master's degree (Nursing, Health Administration, Business Administration) preferred, an RN license or LCSW required, hospital discharge planning experience required, and CCM or ACM preferred.
