Remote Care Coordinator for CNS Therapy Access & Adherence

Praxis Precision Medicines is seeking a Care Coordinator to serve as the single point of contact for patients, caregivers and providers, guiding the full patient journey from enrollment through adherence. You will manage caseloads with autonomy, resolve access barriers, and ensure timely task completion across PSP systems. In this role you will collaborate with specialty pharmacies, reimbursement partners, and field teams; you will document interactions in Salesforce, educate patients on #J-18808-Ljbffr Praxis Precision Medicines, Inc.
