Make a DIFFERENCE for Others, Yourself, & Your Career! $3,500 Sign On Mon-Fri 40 hrs/wk Responsibilities: Serve as primary contact person and coordinator of medical supports for all assigned individuals Advise the program director and other DH provider staff of any medical problems that may hinder a member’s participation in DH or in a specific activity. Keep Program Director informed of changes in health and status of all individuals. Ensure that all paperwork is up to date, including annual physicals, doctor’s orders, ER folders, medical protocols, daily notes, monthly notes, semi-annual and biennial notes, etc. Complete all required documentation on individuals assigned, including day habilitation and HCSIS requirement Requirements Licensed Practical Nurse with one year experience. Some experience with people with intellectual or developmental disabilities or acquired brain injury is ideal. Current license in the State of Massachusetts.
