As a Pharmacist, a typical day might in the family practice department include:

Dispenses medications prescribed by physicians and other health practitioners, which includes providing proper counseling to patients.

Communicates individualized care plans effectively with patients and healthcare providers.

Provides patient care services such as medication therapy management and disease management.

Conducts wellness and preventative health programs.

Ensures compliance with all relevant laws of the applicable State Board of Pharmacy.

Administration of immunizations as allowed by State Board of Pharmacy.

Provides patient-centered care that will improve clinical outcomes and reduce total cost of care.

Supports patients in their initiation and maintenance of new therapy; providing education, optimizing utilization and establishing adherence.

Identify and take specific corrective action for drug-induced problems.

Monitor patients for appropriateness and response to therapy.

Order and analyze laboratory and diagnostic test data to modify drug therapy and dosing as necessary.

Proper documentation in patient's EHR.

Engages patients at clinically relevant times to deliver impactful clinical pharmacy services.

Maintains department performance levels for quality and productivity.

Identifies opportunities for the development of new clinical programs; takes an active role in the development and implementation of clinical projects.

Performs other projects, work, and activities as assigned to ensure continuous service to our patients.

Qualifications needed:

Minimum Bachelor's Degree in Pharmacy required.

Current Idaho State Board of Pharmacy license in good standing.

Certified immunizer.

Experience with the 340B program highly beneficial.

Experience in an ambulatory or clinical setting highly beneficial.

1+ years of Retail pharmacist experience desired.

1+ years of CHC pharmacy experience desired.

Experience in a high volume pharmacy beneficial.

Ability to maintain strict corporate confidentiality and HIPAA compliance.
