LPN/LVN - LTC

Tryfacta is seeking a LPN/LVN - LTC for our client in Hancock, MI 49930. This is a 13-week contract assignment. If you meet the qualifications listed below and are interested, please apply now!

Location: Hancock, MI 49930

Duration: 13 weeks (possible extension)

Work Schedule: 5x8-hour shifts from 2:45 PM to 10:45 PM and 10:45 PM to 6:45 AM

Responsibilities for this position include, but are not limited to:

• Provide direct nursing care to residents in a skilled nursing and long-term care setting.
• Administer medications, treatments, and therapies as prescribed by physicians.
• Monitor and assess resident conditions, reporting any changes promptly to the RN or provider.
• Perform medication passes accurately and safely.
• Maintain accurate and timely nursing documentation.
• Assist with admissions, transfers, and discharges as needed.
• Collaborate with physicians, RNs, CNAs, and other healthcare professionals to develop and implement individualized care plans.
• Ensure compliance with facility policies, infection control standards, and patient safety protocols.
• Provide compassionate care while promoting resident dignity and quality of life.
• Supervise and delegate appropriate tasks to Certified Nursing Assistants (CNAs).
• Respond promptly to emergencies and changes in resident condition.

To be considered for this position, you should have:

• Active Michigan (MI) LPN license (required).
• Current BLS certification.
• COVID-19 vaccination or approved exemption required.
• Previous long-term care (LTC) or skilled nursing facility experience preferred.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work independently and as part of a multidisciplinary team.
