Licensed Practical Nurse

Responsible for assisting the physician in clinical care of all patients to include patient assessment, planning, intervention, and evaluation. Will also have general responsibilities to assist in the day-to-day clinical operations of the office.

Experience: No prior work experience required

Licenses and Certifications

Georgia Licensed Practical Nurse or Multistate Nursing License

Qualifications

Six months previous work experience in a clinical setting as a LPN preferred. Medical practice preferred. Clinical rotation time considered.

Must possess the sense of discipline to work in accordance with accepted clinical standards.

Must maintain high degree of confidentiality.

Must possess the cognitive ability to understand patient's medical records, physician's orders and medical orders to be communicated to patient, hospital, and or other physician.

Must be capable of dealing tactfully and effectively with patients, family members, other employees and physicians.

Prior experience performing oncology tests & labs preferred.

NOTE: BLS within 1 month

Functions

Area of Responsibilities

Asks patients about care obtained outside the practice. Assesses racial and ethnic diversity of its population. Provides interpretation or bilingual services to meet the language needs of its population. Participates in a structured communication process, i.e. huddles.

Identify and Manage Patient Population: Collect and review clinical data i.e., problem list, allergies. BP, height and weight, tobacco use, current medications. Completes comprehensive health assessment. Provides input for three different preventive care services. Provides input for three chronic care services.

Plan and Manage Care: Conducts pre-visit preparations. Provides a written plan of care for the patient family. Assesses and addresses barriers when the patient has not met treatment goals. Identifies patients who may benefit from additional care management support. Reviews and reconciles medication with patient's families. Provides information about new prescriptions. Assesses understanding about medication and barriers to adherence. Documents over the counter medications, herbal therapies, supplements.

Home monitoring of coumadin patients.

Access ports and PICC lines. Shows proficiency with central lines by successfully drawing blood according to P&P. Shows proficiency with venipuncture by successfully drawing blood according to P&P.

Assisting with in office procedures and ordering of oncology specific testing such as bone marrow biopsies, genetic testing, peripheral, and ancillary testing.

Working closely with NGOC research staff on clinical trial guidelines and collection of research labs for patients enrolled in protocols.

Checks patient insurance status prior to venipunctures for proper billing information.

Assembles appropriate equipment prior to venipuncture, including choosing appropriate lab tubes.

Measure and Improve Performance: Participates in improved performance for three preventive care measures. Participates in improving performance for three chronic care measures. Participates in implementing and demonstrating continuous quality improvement.

Competently works in Moderate Complexity Lab processing.

Knowledge of chemotherapy and other treatment plans. Ability to educate patients on potential side effects and outcomes. Ability to recognize side effects and take appropriate steps to notify provider.

Maintain current knowledge of oncology-related medications, treatments, care processes, etc. Utilize Epic Beacon module appropriately to ensure complete and accurate documentation.

Compliance Statement

Employee performs within the prescribed limits of Tanner Health System's Ethics and Compliance program. Is responsible to detect, observe, and report compliance variances to their immediate supervisor, the Compliance Officer, or the Hotline.
