Key Responsibilities

• Provide speech-language pathology services to home health patients across an 11-county territory spanning two office locations
• Evaluate and treat patients with diagnoses including CVA and Aphasia
• Manage a caseload of approximately 6 patients per day in a home health setting
• Travel throughout the assigned service territory to conduct in-home patient visits
• Maintain professional documentation and reporting using provided phone and tablet technology
• Participate in a two-week department-specific orientation with a designated work coach

Required Qualifications

• Active Kentucky state license in hand at time of submission — no pending licenses accepted
• CCC-SLP credential required
• Current BLS certification required
• Minimum of 1 year of clinical experience
• Prior home health experience required
• Ability and willingness to travel across an 11-county territory, with corner-to-corner travel exceeding 2.5 hours

Preferred Experience / Skills

• Experience treating CVA and Aphasia patient populations
• Familiarity with home health documentation and care coordination workflows
