RN Case Manager

Overview Good Shepherd Hospice is seeking a compassionate Registered Nurse Case Manager to serve patients and their families in San Antonio and surrounding communities. The RN will perform nursing assessment, planning, and care to maximize the comfort and health of patients and families. The RN will respond promptly to all phone calls, make patient visits as necessary, and complete after‑hours admissions and deaths as needed. Responsibilities Receive patient assignments from the Patient Care Manager or Case Manager and initiate communication with attending physicians, hospice staff, and other agencies to coordinate optimal care. Maintain regular communication with attending physicians regarding patient/family care. Contact families of new patient assignments and maintain communication as requested. Obtain and incorporate data on physical, psychological, social, and spiritual factors into the plan of care. Maintain up‑to‑date patient records, ensuring accurate documentation of problems, plans, actions, and goals. Instruct patients and caregivers on medication administration, recognizing side effects, performing personal care and oral hygiene, assisting in lifting and ambulation, disaster/safety management, and preparing for end‑of‑life events. Inform the Patient Care Manager of unusual or potentially problematic patient/family issues. Attend hospice team meetings and other patient conferences as required. Provide appropriate support at time of death. Share in providing 24‑hour, seven‑day coverage to patients and families. Evaluate hospice aides every two weeks and evaluate LPNs at least once per month. Through individualized studies, learn and develop an understanding of hospice philosophy and become proficient in pain management through reading, in‑service, or seminars. Perform all other tasks assigned. Qualifications Registered Nurse currently licensed in Texas. Previous experience in hospice, home health, geriatrics, or general medical/surgical nursing. Demonstrated commitment to the hospice philosophy of care. Preferred: experience in hospice, home health, or geriatrics. #J-18808-Ljbffr Good Shepherd Hospice
