Eligibility Specialist

Position Overview Jefferson County invites you to join our Community Assistance Division as a Long‑Term Care (LTC) Eligibility Specialist . In this role you will determine eligibility for Long Term Care Medicaid, Food and Cash Assistance programs and provide exceptional customer service to older adults and individuals with disabilities, ensuring timely access to essential public assistance benefits. Responsibilities Accurately determine eligibility for Long Term Care Medicaid, Food and Cash Assistance programs. Gather, verify, and document financial and non‑financial information required for eligibility decisions. Research complex eligibility factors and make determinations in accordance with federal and state regulations. Manage an ongoing caseload while maintaining regular communication with customers, family members, medical providers, nursing facilities, case management agencies, and community partners. Ensure compliance with state and federal timeliness standards and maintain strict confidentiality of customer data. Conduct effective interviews and communicate clearly in person, by phone, and in writing. Identify and assess customer needs, provide appropriate solutions and referrals, and educate customers on available community resources. Investigate customer concerns and complaints to identify root causes and implement appropriate resolutions. Report potential fraud cases to the Investigations Unit. Troubleshoot system and case issues to ensure accurate case processing. Maintain organized and accurate case documentation and participate in required training. Compensation and Benefits Anticipated Salary: $25.30 USD per hour. Compensation is determined based on education, experience, and skills. Work Schedule This position offers a hybrid schedule with options of five 8‑hour days (Monday‑Friday) or four 10‑hour days. Schedule arrangements may adjust based on operational and business needs. Qualifications Minimum Qualifications One year of related experience. High School diploma or GED. Equivalent combination of education and experience is acceptable. Preferred Qualifications Experience determining eligibility in Medicaid and SNAP benefits using the Colorado Benefits Management System (CBMS). Experience in customer service and de‑escalation techniques. Data entry experience in complex computer databases. Experience in a social/human services field. Spanish‑speaking skills preferred but not required. Skills and Qualities for Success A genuine passion for helping community members and aligning with the County’s mission, vision, and values.
