Physical Therapist for Home Care

University Home Care Inc is seeking a dedicated and compassionate Physical Therapist for Home Care to provide high-quality rehabilitation services to patients in their homes in the Southeast MI Region area. Must be willing to travel. The ideal candidate will use their expertise to develop and implement personalized treatment plans that improve patients' mobility, reduce pain, and enhance overall quality of life. Responsibilities Evaluate patients’ physical conditions and develop individualized treatment plans. Provide hands‑on therapy and guide patients through exercises to improve strength, mobility, and function. Monitor patient progress and adjust treatments as necessary. Educate patients and their families on therapeutic techniques and preventive care. Document all treatments, patient progress, and communications in compliance with company and regulatory standards. Collaborate with other healthcare professionals to coordinate care and optimize patient outcomes. Requirements Licensed Physical Therapist in the state of Michigan. Previous experience in home care or outpatient physical therapy preferred. Strong interpersonal and communication skills. Ability to work independently and manage a patient caseload effectively. Valid driver’s license and reliable transportation for home visits. CPR and First Aid certification. Benefits Competitive hourly compensation ranging from $70.00 to $90.00, paid per visit.
