Job Description Summary The Clinical Pharmacist is a licensed professional responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with company policies, pharmacy practice standards, and applicable regulatory entities. This entry‑level position in the alternate setting coordinates patient services with nursing, monitors progress toward goals, and educates patients, caregivers, staff and other professionals regarding appropriate medication use. Job Responsibilities Determines the suitability of individual patients for home care, collecting and organizing all patient‑specific information needed to assess suitability in accordance with Option Care policies. Designs, recommends, monitors, and evaluates patient‑specific pharmacy care plans for home‑care patients, making suitable recommendations for modifications in prescribed medication therapy or monitoring parameters when appropriate. Modifies the pharmacy care plans based on evaluation of monitoring data and other pertinent patient‑specific information; competently uses pharmacokinetic principles and assesses laboratory values. Provides medication‑use education to Option Care staff, other professionals, home‑care patients, and/or their caregivers using effective patient education techniques. Ensures continuity of pharmaceutical care to and from the home and other patient‑care settings, preparing and dispensing medications with accurate techniques, calibrating equipment, and maintaining safety and quality assurance practices. Manages the use, maintenance, and troubleshooting of medication administration and related equipment, and participates in the reporting of medication‑related incidents and equipment errors. Provides concise and timely responses to drug‑information requests from health care providers and patients, formulating responses based on literature analysis and attending educational seminars to promote professional competency. Manages investigational drug products according to established regulations, utilizing pharmacy support personnel effectively and prioritizing workload to ensure accuracy. Complies with accreditation, legal, regulatory, and safety requirements in all activities. Maintains confidentiality of patient and proprietary information, observing legal and ethical guidelines for safeguarding confidentiality. Supervisory Responsibilities No supervisory responsibilities. Basic Education and/or Experience Requirements Graduate of an accredited School of Pharmacy. Current, active Pharmacy license as granted by the State Board of Pharmacy. One of the following: At least one year of experience in sterile IV admixture procedures. Knowledge of non‑sterile compounding, immunizations and medication therapy management (MTM). Completion of an ASHP accredited Pharmacy Residency Program. Completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy. Equivalent experience as approved by the Vice President (VP) of Pharmacy and/or Senior Vice President (SVP) of Clinical Services. Basic Qualifications Experience providing customer service to internal and external customers, meeting quality standards and evaluating customer satisfaction. Basic level skill in Microsoft Word, Excel, and PowerPoint, including document editing, formatting, and basic spreadsheet operations. Travel Requirements Willing to travel up to 5% of the time for business purposes (within state). Preferred Qualifications & Interests Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy. Experience in home care, hospital or other clean‑room setting. Management Requirements At least one year of indirect management experience of team members, including assistance in development, training, and assignment of work to other team members. Compensation Salary range: $97,113.10 – $161,862.67. Salary determined by education, experience, knowledge, skills, abilities, internal equity and market data.
