Job Description
The Licensed Clinical Social Worker Case Manager (LCSW-C) coordinates and facilitates care, services, and health education for targeted member populations with chronic conditions or high-cost specialty care needs. This role applies advanced knowledge of complex case management processes to ensure member goals are achieved within established timeframes based on individual needs and realistic expectations, while maintaining thorough documentation of outcomes.

Requirements
Education: Master's Degree in Social Work
Work Experience: Case Manager – Minimum of three (3) years of clinical practice experience and three (3) years of case management experience required. Managed care experience preferred.
Salary Range: Minimum 32.40/hour - Maximum 53.48/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
