## DescriptionThe Center at CenterPlace is Greeley's premier, dedicated short-term rehabilitation facility featuring 54 private suites, state of the art rehabilitation equipment, and care delivered in a hotel-like setting.Our center is seeking a team-oriented Physical Therapist for **Evaluations only** who is dedicated to patient outcomes and customer service.**Essential Job Duties*** The Physical Therapist is responsible for the evaluation, plan of care, treatment, re-evaluation, discharge, and appropriate communications to patients and customers.* Develop treatment plans, supervise assistants and aides, contribute to case management, and provide quality patient care.* Maintain a positive level of interaction with patients, peers, and families.* Enhance and expand client relations with facilities and their staff.**Qualifications*** Must be a graduate of an accredited school for Physical Therapy.* National and state licensure REQUIRED to start.
