Description: Admit-to-Discharge Therapist – PRN $55/hr | Outpatient + Home Care | Flexible Weekdays

Community Physical Therapy (CPT) is hiring PRN therapists for our Admit-to-Discharge model, offering maximum schedule flexibility and the ability to follow patients from evaluation through discharge.

Requirements:

• Active Illinois therapy license
• Experience in outpatient or home care helpful
• Willingness to learn TheraOffice/Homecare Homebase
