You'll meet with patients and families, provide emotional support, and guide them through the hospice journey. This Social Worker role includes collaborating with nurses and the care team, advocating for patient needs, and ensuring comfort during critical weekend moments. Every Hospice Social Worker brings calm, clarity, and care when it matters most.What You'll Do (Hospice Social Worker Responsibilities)Complete psychosocial assessments in hospice careDevelop care plans and provide counseling as a Social WorkerCoordinate community resources and referralsSupport advance care planning (DPOA-HC)Maintain timely, accurate documentationCollaborate with the interdisciplinary hospice teamEnsure compliance with Medicare and Illinois standardsSchedule (Weekend Role)Saturday & Sunday12-hour shifts (8:00am – 8:30pm)Consistent weekend schedule for work-life balanceWhat You Bring (Requirements)Master's Degree in Social Work (MSW)Illinois LSW or LCSW preferred1+ year experience as a Social Worker (healthcare or hospice preferred)Strong communication, empathy, and organizational skillsValid driver's license and reliable transportationLocal residence with ability to respond within 30 minutes of assigned service area.
