OTR | Occupational Therapist - Homecare

Join a dedicated healthcare team in the Midwest region as a Homecare Occupational Therapist (OT). In this role, you will develop personalized care plans following thorough patient evaluations to meet both patient and physician goals. You may work with home health or hospice patients, collaborating closely with interdisciplinary teams to ensure comprehensive care.

Essential Duties and Responsibilities

• Evaluate patients for home health admission based on CMS or payer guidelines and agency policies.
• Perform initial and comprehensive assessments within required timeframes, documenting accurately.
• Create individualized, age-appropriate care plans focused on self-management with measurable goals and interventions.
• Update care plans promptly to address new or resolved issues.
• Communicate patient progress and any emerging concerns with physicians and care team members.
• Include patients and caregivers in care and discharge planning, tailoring education to their learning preferences and barriers.
• Manage care delivery efficiently, including scheduling and resource utilization.
• Complete all documentation and revisions within designated timeframes.
• Supervise Certified Occupational Therapy Assistants (COTA) and homecare assistants as applicable, ensuring alignment with care plans.
• Collaborate with multidisciplinary teams through meetings and direct communication.
• Assist patients in obtaining necessary equipment and initiate discharge planning upon admission.
• Provide treatments according to provider orders, adhering to infection control and hand hygiene protocols.

Education

Graduation from an accredited Occupational Therapy program is required.

Experience

At least one year of experience in a JCAHO-accredited hospital and one year in an acute rehabilitation setting serving adults is required. Six months of home care experience is preferred. Computer skills are a plus.

Certification and Licenses

• Current certification from the American Occupational Therapy Association.
• Valid Occupational Therapist license in the relevant state.
• American Heart Association CPR certification.
• Current driver's license and automobile liability insurance.

Mental and Physical Requirements

Ability to stand and walk for extended periods, navigate stairs, and communicate effectively both orally and in writing. Strong interpersonal and organizational skills are essential. Must be able to safely operate a vehicle for 2-4 hours daily and maneuver around home architectural barriers. Tolerance for environmental factors such as temperature changes, smoke, fumes, dust, and pets is necessary.

Working Conditions

Exposure to communicable diseases, infectious materials, and potentially hostile individuals may occur. Work involves handling sharps and biohazardous materials and adapting to various community environments.
