Position Requirements

• Education: Graduate of an accredited nursing program.
• Licensure: Current Registered Nurse (RN) license in Minnesota.
• Experience: Minimum of two years as an RN; hospice or palliative care experience strongly preferred.
• Skills:
  • Independent work and consultation as needed.
  • Supervising home health aide team members.
  • Crisis intervention, grief support, and advance care planning.
  • Acceptance of diverse backgrounds, flexibility, and strong interpersonal skills.
  • Effective time management and attention to detail.

Key Responsibilities

• Formulate the individualized plan of care and provide comprehensive nursing assessments to ensure the highest quality care.
• Provide timely communication and documentation of patient status and care interventions.
• Maintain positive relationships with community referral partners.
• Promote Lake Valley Hospice services with professionalism and care.
• Participate in the after-hours clinical on-call rotation.

• Valid driver's license, auto insurance, and reliable transportation.
