In-Home RN Case Manager for Elder Care
PACE
PACE Southeast Michigan (PACE SEMI) is seeking a Registered Nurse Case Manager to coordinate care for frail elders in the community and home setting. You will assess needs, plan care, and collaborate with a multidisciplinary team to improve outcomes and maintain independence. Responsibilities include supervising staff, teaching participants and families, administering medications per orders, and supporting quality improvement initiatives in alignment with CMS and PACE standards. #J-18808-Ljbffr PACE
