Our organization is currently looking for a full-time, Board-Certified or Board-Eligible physician specializing in Family Practice or Internal Medicine to join our outpatient clinic located in Fairview, Oregon. This role will be designated as the Primary Care Clinical Practice Manager Physician. As part of the Portland VA Medical Center, you will deliver services within a comprehensive network of care facilities and a range of outpatient clinics and hospitals throughout Oregon. The primary care provider will operate as a highly skilled, independent practitioner in patient management, collaborating with an interdisciplinary care team and utilizing cutting-edge medical technologies.
Applicants must hold board certification in either Family Practice or Internal Medicine and exhibit strong leadership and administrative skills. The Health Care Clinic follows the PACT Model (Patient Aligned Care Team). In collaboration with a team that includes a Primary Care Provider (PCP), Registered Nurse (RN), Licensed Practical Nurse (LPN), and a Medical Support Assistant, the PCP will manage a patient panel of approximately 900 individuals. The PACT team delivers comprehensive care to our Veteran population, addressing both acute and chronic conditions. Common health issues include diabetes, hypertension, chronic obstructive pulmonary disease, degenerative arthritis, depression, congestive heart failure, and chronic pain. This position operates Monday through Friday, with no weekend or holiday shifts required.

Work Schedule:
Full-time, Monday to Friday (minimal on-call for abnormal lab results)
Requirements:
- Must be a U.S. Citizen
- Possess a current, full, and unrestricted medical or surgical license in the U.S.
- Board Certification in Family Practice or Internal Medicine
