Social Worker – Part-Time Westchester Behavioral Health Outpatient Clinic

As a Social Worker in the Adult Outpatient Clinic, you will provide direct evidence-based care for adult patients with diverse mental health conditions. You will work collaboratively with psychiatrists, psychologists, psychiatric nurse practitioners, and other healthcare professionals to ensure integrated and patient-centered care. Other key clinical responsibilities include:

• Provide time-limited individual, group and family therapy using evidence-based practices tailored to the needs of the clinic's patient population
• Develop individualized treatment plans
• Collaborate with an interdisciplinary team to coordinate patient care
• Educate patients and families about mental health diagnoses, treatment options and strategies for emotional well-being
• Monitor and evaluate patient progress and modify interventions as needed
• Maintain accurate and timely documentation in compliance with hospital and regulatory standards
• Participate in case conferences, team meetings, and quality improvement initiatives
• Support crisis intervention efforts and risk assessment as needed

This is a part-time position, with coverage for 21 hours/week. Monday - Friday. No weekend coverage is available.

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.
