Admissions Nurse

This position will be an Admissions nurse with a case load responsible for the Baltimore City territory. Responsible for providing professional nursing care to patients following established standards and practices in the home setting. Functions as primary clinical liaison between patients and physicians. Completes admission visits OASIS documentation and other documentation as needed for new and existing patients.

Essential Functions:

• Completes the admission process appropriately for skilled services per regulatory standards and per the policies and procedures of HomeCare Maryland.
• Completes thorough and accurate OASIS data and plan of care.
• Performs initial and ongoing assessment of physical, psychosocial, and environmental needs of patients in the home setting.
• Confirms assessment findings with physician at the start of skilled services.
• Develops therapeutic nursing interventions to meet patient's individual goals and determines the need for additional skilled services.
• Provides supervision for home health aide and licensed practical nurses as appropriate per the policies of HomeCare Maryland LLC and the Maryland regulatory agency.

Education and Experience:

• Registered Nurse License for the state of Maryland.
• Current BLS for Healthcare Providers.
• Prior experience in home care preferred, positive attitude a must!
• 2-3 years as a Registered Nurse.
