Job TitleLocation: 75% Travel to members' homes, ideally located in Central KS - Salina, Hutchinson, Great Bend, Pratt, and surrounding areasDuration: 6 months, possible extensionShift: 8am-5pm, with possibility of some evening hours due to business needResponsibilitiesSupport members in foster care and coordinate services to ensure their needs are metConduct 48-hour in-person visits following a placement change for the memberParticipate in 7-day team meetings following a placement change for the memberConduct telephonic outreaches based on member's acuityPerform in-person visits based on member's acuityComplete annual assessmentsAttend in-person visits that may take place after 5pmParticipate in weekly team meetingsTrack completion and timeliness of Health Risk Assessment and Plan of ServiceRequirementsMaster's degree in Behavioral Health, Social Work, or a degree from an Accredited School of NursingLicensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN based on state contract requirements with BH experienceMinimum of 1 year of experience coordinating services across child welfare and related systems of careStrong communication, high-quality documentation, adaptability/flexibility, and solution-focused approachPreferred SkillsExperience working in child welfare and related systems of care
