Concept Rehab is seeking a compassionate and motivated Occupational Therapist (OT) to join our rehabilitation team in a skilled nursing and post-acute care environment. In this role, you'll help patients regain the skills and confidence needed to safely perform everyday activities, improve functional independence, and maximize quality of life.

Requirements
What You'll Do
- Perform comprehensive occupational therapy evaluations and functional assessments
- Develop individualized treatment plans based on patient goals and clinical needs
- Deliver skilled interventions focused on activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
- Address cognitive, perceptual, sensory, upper extremity, and functional mobility deficits
- Recommend adaptive equipment, environmental modifications, and safety strategies
- Educate patients, families, and caregivers to support successful outcomes
- Collaborate with nursing, physicians, rehabilitation professionals, and facility leadership
- Supervise and support Certified Occupational Therapy Assistants (COTAs) as appropriate
- Participate in interdisciplinary care plan meetings and discharge planning discussions
- Maintain accurate, timely, and compliant documentation in accordance with regulatory requirements
Qualifications
- Bachelor's, Master's, or Doctorate degree in Occupational Therapy
- Current Ohio Occupational Therapist license in good standing
- Skilled nursing, geriatric, or post-acute rehabilitation experience preferred
- Strong clinical reasoning and problem-solving abilities
- Excellent communication and interpersonal skills
- Commitment to patient-centered care and positive clinical outcomes
