Physical Therapist - Home Health

Clinical Magnet, a division of ICON Medical Network, is seeking a compassionate and motivated Physical Therapist for a full-time, permanent Home Health opportunity in the New Haven, Connecticut  area - coverage flows from New Havcen to Branford, Guilford, Madison, and the Connecticut shoreline. Responsibilities

• Evaluate and treat patients in the home health setting
• Develop individualized treatment plans based on patient needs and goals
• Monitor patient progress and adjust care plans as appropriate
• Collaborate with physicians, nurses, and interdisciplinary care teams
• Complete timely and accurate documentation
• Educate patients and caregivers on therapeutic techniques and safety

Qualifications

• Graduate of an APTA-accredited Physical Therapy program
• Active Connecticut PT license
• Minimum of 1 year of clinical experience preferred
• Home health experience preferred but not required
• Valid driver’s license, reliable transportation, and current auto insurance
• Ability to travel throughout the assigned territory
