Physical Therapist PT Home Health

Empath Home Health , a member of Empath Health is seeking a compassionate Home Health PT (Physical Therapist) to work with our Home Health team to deliver individualized care to patients in their homes in North Sarasota county .

Locations : Home health visits in North Sarasota county (including North Sarasota, Downtown Sarasota, Longboat Key, University Parkway, Fruitville, and surrounding areas)


Schedule: M-F 40 hours with weekend/holiday on-call rotations

What you'll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled physical therapy services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need

• Active Florida PT license
• Minimum of 1 year experience as a licensed Physical Therapist (PT).
• Home health care experience strongly preferred.
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on-call rotations
