Position Overview We are hiring a Case Manager to enhance the independence, well‑being, and happiness of the individuals we serve. In this role, you will receive comprehensive training to thrive in this rewarding field. Key Responsibilities Deliver personalized case‑management services, including the development and implementation of Individual Support Plans (ISP) and Person‑Centred Plans (PCP). Coordinate essential services such as medical, dental, mental health, housing, and community integration. Ensure compliance with federal and state regulations and uphold 3Rivers’ policies and procedures. Maintain precise records, including Initial Assessments, PCPs, ISPs, and communication documentation. Foster collaborative relationships with families, agencies, and involved parties. Participate actively in agency meetings, training sessions, and goal planning for the case‑management program. Represent 3Rivers positively within the community and stay informed about developments in the field. Benefits Starting pay $22.50 (pay commensurate with experience). Sign‑on bonus of $1,000 payable in two installments: half at 90 days of service and the remainder at 6 months. Health, dental, and vision insurance. Strong reading and writing skills. Valid and insurable Maine driver’s license and reliable transportation. One year of relevant experience, with three years of experience working with individuals with developmental disabilities preferred. Experience with Evergreen and Therap preferred.
