Salary:
Pinnacle Hospice is seeking an After-Hours Hospice LVN to provide skilled, on-call nursing care outside of standard business hours, throughout the Sacramento Area (ROSEVILLE, ROCKLIN, AUBURN, LINCOLN, FOLSOM).
Qualifications
- Current LVN license in good standing
- Strong clinical judgment and ability to work independently
- Excellent communication and problem-solving skills
- Compassionate presence in emotionally sensitive situations
- Valid drivers license
- Reliable, insured vehicle in good working condition (per state and organizational requirements)

Responsibilities
- Hospice Afterhours RN/LVN (per diem/oncall)
- Be there when patients and families need you most
- Pinnacle Hospice is seeking an After-Hours Hospice LVN to provide skilled, on-call nursing care outside of standard business hours
- The After-Hours Hospice LVN provides professional nursing care to hospice patients during evenings, nights, and weekends
- This role ensures continuity of care, symptom management, and emotional support for patients and families from 5:00 PM - 8:30 AM
- The nurse works independently while maintaining close communication with the interdisciplinary team, physicians, and leadership to support individualized plans of care
- Respond to after-hours patient calls and urgent care needs
- Perform skilled nursing assessments and symptom management
- Provide emotional support and education to patients and families
- Implement and update patient plans of care as needed
- Communicate patient condition changes to the interdisciplinary team
- Collaborate with physicians and caregivers regarding patient needs
- Complete accurate and timely clinical documentation
- Uphold hospice philosophy, policies, and regulatory standards
