Family Medicine PhysicianCommunity Memorial is seeking a Board-Certified or Board-Eligible Family Medicine Physician to join a well-established primary care practice in the picturesque community of Hamilton, New York. This is an excellent opportunity for a physician seeking a collaborative outpatient practice environment with strong clinical support, a loyal patient base, and a commitment to delivering high-quality, patient-centered care.The Family Medicine Physician will provide comprehensive care to patients of all ages, serving as the primary point of contact for preventive care, chronic disease management, acute illnesses, and overall health maintenance.Comprehensive Primary CareProvide full-spectrum family medicine services in an outpatient setting.Diagnose and treat acute illnesses, injuries, and chronic medical conditions using evidence-based medical practices.Develop individualized treatment plans tailored to each patient's medical history, lifestyle, and healthcare goals.Conduct annual wellness visits, routine physicals, preventive screenings, and immunizations.Perform comprehensive patient evaluations and clinical assessments.Chronic Disease ManagementManage chronic conditions through ongoing monitoring, medication management, and patient education.Coordinate care plans designed to improve long-term patient outcomes and quality of life.Monitor treatment effectiveness and adjust care plans as needed.Diagnostic & Preventive MedicineOrder and interpret laboratory studies, diagnostic testing, and screening exams.Review diagnostic findings and consultant recommendations to ensure accurate diagnosis and treatment.Promote preventive healthcare through education, counseling, and risk-factor reduction strategies.Collaborative Patient CareCoordinate care with specialists, hospitals, therapists, behavioral health providers, and other healthcare professionals.Facilitate referrals and ensure continuity of care across the healthcare continuum.Work closely with advanced practice providers, nursing staff, care managers, and support personnel to provide exceptional patient care.Documentation & Quality ImprovementMaintain accurate and timely documentation within the electronic medical record (EMR).Participate in quality improvement, patient safety, and population health initiatives.Ensure compliance with organizational policies, regulatory requirements, and clinical best practices.QualificationsMD or DO degree from an accredited medical school.Board Certified or Board Eligible in Family Medicine.Eligible for or currently holding an unrestricted New York State Medical License.Current BLS, ACLS, and PALS certifications preferred.Strong clinical judgment and commitment to evidence-based medicine.Excellent communication and patient relationship-building skills.Commitment to delivering compassionate, patient-centered care.Compensation & BenefitsBase Salary: $250,000 - $290,000Additional earnings through a wRVU productivity incentive planComprehensive benefits packagePaid CME and professional development supportMalpractice coverageRetirement savings optionsRelocation assistance available for qualified candidatesWhy Hamilton, NY?Nestled in Central New York and home to the prestigious Colgate University, Hamilton offers small-town charm with access to outdoor recreation, excellent schools, vibrant local shops, restaurants, and a welcoming community.
