Social Work Manager

Compensation & Schedule $15,000 Sign‑On Bonus or Student Loan Assistance! Hybrid schedule: 3 days in office / 2 days remote. About the Role Our groundbreaking hospice and palliative care programs offer a significant difference when dealing with a life‑limiting condition. We provide a broad range of community or facility‑based services to help patients and their families ease the burden of managing a life‑limiting illness or end‑of‑life care. Responsibilities Mentor and provide leadership direction to the Social Work staff. Ensure Social Worker documentation compliance. Participate in referral and community education and presentations. Oversee the preparation and planning of weekly team meetings. Coordinate the provision of psychosocial services and maintain adequate staffing and appropriate patient assignments. Integrate psychosocial services as part of the interdisciplinary team approach. Serve as a member of the Hospice and Palliative Care management team and attend management meetings as directed. Qualifications Master’s Degree in Social Work from a Graduate School of Social Work accredited by the Council of Social Work Education. Minimum five years post‑Master’s; minimum 2 years end‑of‑life care experience; minimum 2 years supervisory/program development experience.
