Key Responsibilities: Prepare and dispense drug orders according to established policies and protocols. Accurately interpret and transcribe drug orders into the computerized patient medication record. Compounding and dispensing both non-sterile and sterile products, chemotherapy, and parenteral nutrition with precision. Efficiently issue controlled substances to patient care areas while complying with legal requirements and institutional policies. Maintain current pharmacist licensure and participate in staff meetings to ensure continuous improvement. Supervise pharmacy support personnel and oversee the daily activities of pharmacy technicians. Provide ambulatory pharmaceutical care services, including managing patient drug therapy, anticoagulation management, and patient education. Clinical Duties: Ensure safe and cost-effective drug therapies for patients according to established guidelines. Monitor drug therapy regimens for contraindications, interactions, allergies, and appropriateness. Assist with pharmacokinetics consult services and renal drug dosing as needed. Detect and report any suspected adverse drug reactions in a timely manner. Participate in maintaining the hospital drug formulary and promoting rational drug therapy selection. Deliver accurate drug information to healthcare professionals and educate patients and families per institutional protocols. Engage in quality improvement and medication use review activities to enhance departmental performance. Participate in interdisciplinary rounds to collaborate on patient care.
