Registered Nurse - PRN - Varies - Hospice & Palliative Care

Position Summary Registered Nurse – PRN – Hospice & Palliative Care. Responsible for delivering nursing services to terminally ill patients in their homes and providing supportive services to families, complying with federal, state and local health and safety laws, and hospice policies. Qualifications Graduate of an accredited School of Nursing. BSN preferred. Current RN license in Texas. Current Basic Life Support (BLS) certification. Valid current Driver’s License and proof of Auto Insurance. Minimum one year of acute‑care nursing experience. Preferred two years of hospice or home‑health experience. Knowledge of local, state and federal hospice regulations. Demonstrated ability to collaborate with physicians, health‑care professionals, patients, families, and interdisciplinary team members. Job Knowledge & Experience Documentation of at least one year acute‑care experience and preferred two years of hospice or home‑health experience. Responsibilities Administer medications, monitor interactions and side‑effects, and manage pain and symptom control. Coordinate care plans and refer to additional services as needed. Prepare and submit clinical and progress notes in accordance with policy and procedure guidelines. Communicate information using current technology. Supervise LVN, CNA and paraprofessionals; evaluate performance and provide corrective action. Provide hospice training to staff, family members and caregivers. Continuously evaluate patient and family response to care and adjust plans. Assess caregiver capacity and identify support systems. Handle medication ordering, inventory management and cost containment. Ensure patient privacy and adhere to confidentiality policies. Participate in clinical calls, QA/PI processes, departmental meetings, orientation and in‑services. Maintain current competency and skill levels. Act as physician agent for prescription calls following agency protocols. Apply admission and recertification criteria for hospice patients. Manage grief risks, cultural or speech needs, and communicate patient conditions to the team. Work hours Monday‑Friday 8:00 AM–5:00 PM with rotating on‑call after‑hours and weekends. Perform initial and ongoing assessments of disease impact, functional, psychosocial and environmental needs. Other duties as assigned. Physical Requirements Light/moderate lifting up to 20 lbs. from floor to shoulder height. Ability to kneel, assist lifting more than 20 lbs., walk and carry up to 20 lbs. Perform repeated bending, reaching above shoulder, stepping and pulling. Climbing ladders – not required. Use of hand‑over‑hand pulling, pushing and twisting. Operate office equipment, mechanical equipment, read/write, count, comprehend written and verbal communication. Operate personal vehicle. Manage stress. Working Conditions Over 95% of time in an air‑conditioned environment with occasional exposure to noise, minimal exposure to malodorous or infectious body fluids, and low risk of blood or bodily fluid contact.
