Responsibilities
- Perform comprehensive physical therapy evaluations on residents referred for rehabilitation.
- Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
- Provide gait training, therapeutic exercises, manual therapy, and modalities as indicated by clinical assessment.
- Monitor progress, document outcomes, and adjust care plans in collaboration with the interdisciplinary team.
- Educate residents and families on home exercise programs and progression expectations.
- Maintain accurate, timely documentation in accordance with regulatory and facility standards.

Requirements
- Active Physical Therapist license to practice in Pennsylvania or eligibility for licensure by endorsement.
- Doctor of Physical Therapy (DPT) degree or equivalent.
- Strong clinical skills in geriatrics and rehabilitation within a skilled nursing environment.
- Ability to work in a fast-paced, team-based setting with an emphasis on patient safety and quality of care.
- Excellent documentation, communication, and patient education abilities.
- Capacity to work a typical 08:00–16:30 schedule and meet contract requirements for 12-week assignment.
