Responsibilities

• Evaluate patients with orthopedic, neurological, and mobility-related conditions.
• Develop and implement individualized treatment plans.
• Improve strength, balance, gait, endurance, and overall functional mobility.
• Monitor patient progress and modify interventions as needed.
• Complete timely and accurate clinical documentation.
• Collaborate with physicians, nurses, therapists, and caregivers.
• Educate patients and families on rehabilitation goals and home exercise programs.

Qualifications:

LICENSURE, CERTIFICATION AND EDUCATIONAL REQUIREMENTS

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist (PT) license in the state of practice.
• New graduates welcome where applicable.
• Experience in skilled nursing, rehabilitation, home health, or post-acute care preferred.
• Strong clinical, communication, and documentation skills.
• Foreign-trained applicants must possess credentials deemed substantially equivalent by the applicable credentialing organization.
