Care Management Support Specialist
CenterWell Senior Primary Care
CenterWell Senior Primary Care is seeking a Care Management Support Assistant to provide non-clinical support for member assessments and resources, reporting to the Associate Operations Director. This role involves routine administrative tasks and supports care management processes to promote member wellness. Typical schedule is Monday–Friday, 8 AM–5 PM, with 40 hours per week.
