LICENSED VOCATIONAL NURSE (LVN)

GENERAL SUMMARY
The LVN will provide direct nursing services for Opioid Addiction and Treatment Services (OATS) Program consumers. LVNs in the OATS assist with laboratory testing, EKGs, TB screenings and other medical treatments, maintain pharmaceutical oversight of medication. The LVN will also complete associated documentation for the consumer and program. LVNs within the OATS clinic will be expected to work a 40‑hour work week including a rotating Saturday schedule. May also work floating schedule to other services with CHCS based upon business needs.ESSENTIAL DUTIES & RESPONSIBILITIES
Essential functions are the basic job duties that an employee must be able to perform, with or without reasonable accommodation. Administers medications and provides medication education to patients. Assists with laboratory collection and other medical treatments/interventions. Assists with training new nursing staff in clinic duties in coordination with the RN Manager. Communicates with collaborating physicians or specialists regarding patient care. Communicates with patients regarding treatment plans. Conforms to the Texas Nurse I Practice Act and the Board’s rules and regulations as well as all Federal, State, and local laws, rules, or regulations affecting the LVN’s current area of nursing. Coordinates on medication management and reconciliation with consumers. Coordinates services with pharmacy representatives and/or companies. Educates patients about health maintenance and disease prevention. Ensures the operation of a safe, therapeutic environment. Facilitates referrals to other healthcare professionals and medical facilities. Maintains pharmaceutical oversight of medication samples for specified units participating in the program through assigned unit level staff. Obtains and orders medical records, labs and laboratory results. Monitors various supply inventory and ensuring proper rotation of expiring medications/vaccines/supplies. Participates as a member of the inter‑disciplinary team. Provides administrative support, including answering phones, charting and filing, and ensuring adequate supplies. Provides daily nursing services, including quality nursing care, medication support, educational programs, and case management services for clients. Trains and educates clients on their medical needs. LVNs within the OATS clinic will be required to work a Monday through Saturday work schedule with varied hours depending on business needs. May also work floating schedule to other services with CHCS based upon business needs. Performs other related duties as required. Specialized Programs/Units Opioid Addiction Treatment Services (OATS) Assists in the intake and admission process with new consumers. Assists with observed urine drug screens and other laboratory collections. Coordinated consumers Initial, annual and follow‑up visits with the Doctor Communicates effectively and collaborates with interdisciplinary team. Coordinates and dispenses take‑home medications as approved and prescribed for each consumer. Maintains pharmaceutical oversight of medications in the dispensing unit. Participate in staffing sessions as needed to discuss consumer treatment plans and outcomes. Performs pill counts and reconciliation of any prescribed medications with consumers that could show up on a drug test. Performs visual assessment of consumer’s cognition and motor skills prior to administering dose of controlled substances like Methadone and/or Suboxone and provides medication monitoring and education to consumers. Transports controlled substances to consumers living in residential treatment programs– i.e. Jail, NAS or other residential programs.MINIMUM ENTRANCE QUALIFICATIONS
Education and Experience High School Diploma or GED equivalency is required. Licenses or Certifications Current Texas LVN licensure Other Requirements OATS: Complete all SUD required training. Must maintain a valid driver’s license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company’s auto insurance carrier if you drive your vehicle during company business. Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies. Must be able to meet the physical requirements to complete Nonviolent Crisis Intervention (NCI) and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance. Nonviolent Crisis Intervention (NCI) is a proven safe and harm‑free method of behavior modification. All consumer‑facing employees of CHCS must take NCI courses facilitated by the CHCS Training team within the first 45 days of employment. NCI is intended to support human service professionals in giving aggressive, disruptive, or out‑of‑control people the best care and welfare possible, even in the most violent situation. NCI training provides staff with the skills to safely recognize and respond to everyday crisis situations that may involve more challenging behaviors. It focuses on prevention and offers proven strategies for safely defusing anxious, hostile or violent behavior at the earliest possible stage. Depending on assigned unit/program, applicants for this position must pass a Criminal Justice Information Systems (CJIS) fingerprint‑based background check and maintain CJIS eligibility. Due to CJIS requirements related to system access, the following may result in being disqualified for this position: Felony Convictions, Felony Deferred Adjudication, Class A & B Misdemeanor Deferred Adjudication, Class B Misdemeanor Convictions, an Open Arrest for Any Criminal Offense (Felony or Misdemeanor), and Family Violence Convictions.PREFERRED QUALIFICATIONS
Associate’s Degree in Nursing from an accredited university. Bilingual (English/Spanish) preferred. Language Proficiency Pay (LPP) payments are subject to successful testing, certification by CHCS Payroll, and availability of funding. Funding may be renewed in subsequent fiscal years but is not guaranteed. Previous experience in nursing or other medical fields. One (1) to two (2) years of nursing experience in an acute fast‑paced environment which includes one (1) year in a psychiatric or behavioral health setting.SUPERVISION
Job may oversee duties of a Certified Medical Assistant (CMA) if applicable in clinic. Performing basic mathematical functions such as addition, subtraction, multiplication, division, percentages, and ratios. Performing phlebotomy procedures using proficient techniques that avoid repetitive vein puncture. Ability to: Accurately organize and maintain paper documents and electronic files. Accurately set up exam rooms for doctor or nurse visits. Address concerns from consumers about medication and treatments. Build and maintain rapport with patients. Conduct assessment of patient condition. Effectively communicate, both verbally and in writing, to clients and staff members. Establish and maintain effective working relationships. Maintain accurate and complete records. Maintain the confidentiality of information and professional boundaries. Manage the nursing phone line. Meet schedules and deadlines of the work. Thrive in a fast‑paced environment. Understand and carry out oral and written directions.PHYSICAL DEMANDS
Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in various care. The employee must be able to regularly lift and/or move up to 40 pounds and occasionally must lift and/or move up to 50 pounds. Must be able to meet the physical requirements to complete Nonviolent Crisis Intervention (NCI) and CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance.
