Home Health Physical Therapist Internally at Ascension at Home along with Compassus, you will work one-on-one with patients in their homes, helping them regain strength and independence to participate in daily life activities. Responsibilities Provide therapeutic care, develop individualized treatment plans, and monitor patient progress to enable functional outcomes in home settings.
