Gentiva is seeking a compassionate Social Worker (MSW) to provide home-based psychosocial support to patients and families. In this role, you will complete psychosocial assessments, develop care plans, and coordinate services that promote dignity, comfort, and quality of life. You will collaborate closely with an interdisciplinary team, connect patients with community resources, support advance care planning, and provide emotional counseling and crisis intervention. Gentiva offers a supportive, mission-driven culture with clinical support, training, and opportunities for growth in home-based care.
