Position Overview We are seeking a compassionate and skilled Home Health Physical Therapist (PT) to provide high-quality, patient-centered care in Kings County, California . This role involves delivering physical therapy services to patients in their homes, focusing on restoring mobility, improving function, and enhancing overall quality of life. The ideal candidate is self-motivated, organized, and committed to delivering evidence-based care in a home health setting. Key Responsibilities Conduct comprehensive patient evaluations, including physical assessments and functional mobility analysis Develop and implement individualized treatment plans based on patient needs and physician orders Provide therapeutic interventions such as: Strengthening and conditioning exercises Gait and balance training Pain management techniques Post-surgical rehabilitation Educate patients and caregivers on home exercise programs, safety precautions, and disease management Monitor patient progress and adjust treatment plans as needed Maintain accurate and timely clinical documentation in accordance with regulatory and organizational standards Collaborate with physicians, nurses, occupational therapists, and other care team members to ensure coordinated care Ensure compliance with home health regulations, safety standards, and best practices Work Schedule & Structure Full-time Flexible scheduling based on patient caseload Travel required within Kings County and surrounding areas Primarily home-based patient care environment Compensation & Benefits Competitive compensation based on experience and employment type Mileage reimbursement or travel allowance Comprehensive benefits package (for eligible employees), including: Health, dental, and vision insurance Retirement savings plan (e.g., 401(k)) Paid time off (PTO) Continuing education support Opportunities for career advancement and professional development Qualifications Doctor of Physical Therapy (DPT) or Master’s degree in Physical Therapy from an accredited program Active California Physical Therapist license in good standing Valid driver’s license and reliable transportation Current CPR/BLS certification Minimum of 1 year of experience preferred (home health experience strongly preferred) Strong clinical, communication, and organizational skills Ability to work independently and manage time effectively Preferred Skills Experience with electronic medical records (EMR) systems Knowledge of Medicare/Medicaid documentation and billing requirements Experience with geriatric and/or post-acute care populations Strong patient education and interpersonal skills Work Environment Home health setting requiring travel to patient residences Independent work with regular interdisciplinary team collaboration Supportive clinical and administrative infrastructure Open to local candidates as well as those willing to relocate
