Field-Based Youth Housing Case Manager

Wellnest seeks an ICMS Case Manager to support Transition Age Youth in South Los Angeles. You will develop and monitor service plans, connect clients to housing, health care and employment, and provide field-based support to maintain housing stability. Requirements include a BA in social work or related field and experience with homelessness, mental health or intensive case management.
