In-Home Nurse Practitioner or Physician Assistant Part Time - Roanoke

Signify Health, part of CVS Health, is seeking a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations, engaging with patients in the comfort of their homes and helping bridge gaps in care. The role involves building trusted relationships, delivering flexible, comprehensive and personalized evaluations at the patient’s front door. Job Highlights The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and additional diagnostic tests (e.g., diabetic eye exams, spirometry) if ordered by the health plan. Clinicians use an iPad and intuitive workflow; prescribing, lab ordering, or changing treatments is not required. Responsibilities Bring your heart into every visit, partnering with a national network of purpose‑driven clinicians. Visit millions of people nationwide, connecting them to the right care for their unique needs. Collaborate with Signify Health as a clinician colleague to fulfill role requirements. Travel locally and within licensed states as needed. Conduct virtual visits when necessary. Provide all indicated Diagnostic & Preventive Services products (spirometry, DEE, bone density screening, etc.). A Note on Our Work Environment As an in‑home clinician, your primary workplace is the private residence of health‑plan members. The environments vary and may contain pets, secondhand smoke, and allergens or irritants. Candidates must be able to perform duties safely and comfortably in such settings. Board certification (required). Multi‑state licenses (preferred). Anticipated Weekly Hours 29.5 hours Pay Range $41.85 – $90.13 per hour (base hourly rate or full‑time salary).
