Responsibilities:

• Conduct comprehensive in-home physical therapy evaluations and assessments for Medicare-eligible patients
• Develop and implement individualized treatment plans to restore function, improve mobility, and reduce pain
• Provide therapeutic exercises, manual therapy, and functional training in the home setting
• Monitor and document patient progress, adjusting care plans as needed to meet goals
• Collaborate with physicians, nurses, and other healthcare team members to coordinate patient care
• Educate patients and caregivers on home exercise programs and fall prevention strategies
• Ensure accurate and timely documentation in compliance with Medicare and agency standards

Requirements:

• Active Physical Therapist (PT) license in the state of Colorado
• Graduate of an accredited Physical Therapy program (DPT or MPT)
• Experience in home health, geriatrics, or Medicare-certified settings preferred
• Strong knowledge of Medicare guidelines and home health documentation requirements
• Excellent communication, time management, and organizational skills
• Valid driver's license and reliable transportation for home visits in the Grand Junction area
• Compassionate, self-motivated, and able to work independently in a field-based role
