About the Role

As a Nurse Practitioner or Physician Assistant, you will provide comprehensive, patient-centered care across post-acute and long-term care settings. You ll collaborate with a multidisciplinary team to improve outcomes, support recovery, and enhance quality of life for your patients.

This is an ideal role for clinicians who enjoy autonomy, continuity of care, and working with a diverse patient population.

Key Responsibilities
  • Provide comprehensive medical care and ongoing patient management
  • Conduct assessments, diagnoses, and treatment planning
  • Manage chronic conditions and acute medical changes
  • Collaborate with physicians and interdisciplinary teams
  • Participate in care planning and team discussions
  • Maintain accurate and timely documentation
  • Focus on patient-centered, quality-driven outcomes
Work-Life Balance
  • Monday Friday schedule (8 AM 5 PM)
  • No evenings, weekends, holidays, or on-call duties

Qualifications
  • Licensed Nurse Practitioner or Physician Assistant
  • Active Oregon license (or ability to obtain)
  • Background in Family, Adult, or Geriatric care preferred
  • Post-acute or facility-based experience is a plus (not required)
