In this role, you will create and manage individualized plans of care, using strong attention to detail, effective communication, and a commitment to acting in the best interest of your patients. Hours Monday-Thursday 12 hr night shift 8pm-8am Must work every other weekend or every third weekend (Friday, Saturday, Sunday) night shift 8pm-8am. Location Savage MN, 55378. Must be willing and able to drive to Scott County MN. Responsibilities Work with both respiratory trach and ventilator and non-respiratory patients in the 1:1 private home setting Providing high-quality patient care in accordance with established protocols and standards. Conducting thorough patient assessments and documenting medical histories. Administering medications and treatments as prescribed. Assisting with critical care tasks, including airway management and acute pain management. Monitoring vital signs and reporting any changes in patient conditions. Educating patients and families on health management and medication administration.
