Facility Setting: Home Health (Office-Based with Occasional Field Visits)
Patient Population: Adult Patients
Schedule

• Monday-Friday
• Standard daytime business hours
• No routine weekends
• No on-call requirements

Position Overview

We are seeking a Director of Nursing to oversee clinical operations, ensure regulatory compliance, support quality improvement initiatives, and provide leadership to a growing Home Health team. This role is primarily office-based with occasional field visits and Start of Care support as needed.
Responsibilities

• Provide clinical leadership and oversight for Home Health operations
• Supervise nurses, therapists, and Home Health staff
• Ensure compliance with Texas HCSSA regulations and CMS Conditions of Participation
• Oversee OASIS assessments, Plans of Care, physician orders, and clinical documentation
• Lead Quality Assurance and Performance Improvement (QAPI) initiatives

Qualifications

• Active Texas Registered Nurse (RN) license
• Previous Home Health Director of Nursing (DON) experience required
• Home Health clinical experience required
• Experience with state and/or accreditation surveys
• Strong knowledge of Texas HCSSA regulations and CMS Conditions of Participation
• Proficiency with EMR systems
• Excellent leadership, organizational, and communication skills

Experience

• Prior Home Health DON experience required
• Survey readiness and compliance experience strongly preferred
• New graduates are not eligible
