- Assist social workers in conducting initial and on-going assessments of patients and residents to identify their physical, emotional, and social needs
- Develop and implement individualized treatment plans to meet the unique needs of each patient and resident
- Provide supportive counseling and therapy to patients and residents, as well as their families and caregivers
- Collaborate with healthcare teams to ensure comprehensive care and support
- Maintain accurate and detailed records of patient and resident care
- Participate in interdisciplinary team meetings to discuss patient and resident progress and develop strategies for improved outcomes

- BACHELORS degree in Social Work or a related field
- 2 years of experience in social work or a related field
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
