RN Care Manager: Transitional & Chronic Care Coordinator

Community Health Center in Mason City, Iowa is seeking an RN Case Manager to coordinate patient care across transitions and manage chronic disease programs. This role requires a current RN license in Iowa and at least 2-3 years of clinical nursing experience.
