Home Health Medical Social Worker-PRN

Become a part of our caring community. The Medical Social Worker, reporting to the Clinical Manager, participates in the interdisciplinary care provided to home health patients. The role involves evaluating and developing a personalized plan of care that addresses the patient’s emotional and social needs, supervising the Social Worker Assistant within that plan, and operating within CenterWell Home Health’s policies, applicable healthcare standards, governmental laws, and regulations. Responsibilities Assess the patient’s social and emotional state in relation to their illness or injury, care needs, and response to treatment. Evaluate relationships between the patient’s medical and nursing needs, financial resources, and available community resources. Communicate with patients, family members, and organizational associates. Provide ongoing assessment of patient and family needs and responses while ensuring understanding of the treatment process and coping with social and emotional responses. Participate as a member of the interdisciplinary care team in care coordination activities and act as a resource to other health team members for the identification and resolution of patient needs. Evaluate the performance of the Social Work Assistant (BSW) to assure compliance with company, government, and professional standards. Maintain and submit required documentation, including case conferences, patient/physician community contacts, visit reports, progress notes, and collaboration with other health disciplines for optimum patient care. Required Qualifications Minimum of one year of experience as a social worker in a health care setting, home health, and/or hospice. Social Worker licensure in the state of practice, if required by state law or regulation. Valid driver’s license, auto insurance, and reliable transportation. Current CPR certification. Experience with discharge planning and obtaining community resources (housing, shelter, funeral/memorial services, legal, information and referral, state/federal financial and medication programs, and eligibility). Knowledge of medications and their correct administration. Demonstrated ability to organize tasks, develop action plans, set priorities, and function under stressful situations. Flexibility in work hours and local travel. Maintain current licensure certifications and meet mandatory continuing education requirements. Proficiency in reading, writing, and speaking fluent English. Additional Information TB Statement: This role is considered patient-facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, TB screening is required. Driving Statement: This role is part of Humana’s driver safety program and requires a valid state driver’s license and maintenance of personal vehicle liability insurance. The individual must carry insurance in accordance with their residing state’s minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, whichever is higher. Scheduled Weekly Hours 40 hours per week Pay Range $53,700 - $72,600 per year (starting base pay estimate; may vary by location and individual qualifications).
