Position: Physical Therapist - Home Care

Location: Manchester, NH 03103

Shift: Days 5x8-Hours (Mon-Fri) with Weekend rotation

Job type: Full-time, Permanent

Pay rates: (based on experience) $37.14 per hour - $59.42 per hour

Bonus: This role also comes with a $10,000 sign on bonus (Paid in 3,6, and 12 months)

Job Summary

We are seeking a compassionate and skilled Physical Therapist to join our Home Care team in Manchester, NH. The Physical Therapist provides in-home physical therapy services to patients of all ages, develops individualized treatment plans, evaluates patient progress, and collaborates with physicians and interdisciplinary care teams to deliver high-quality patient care. This is a full-time, day shift position that includes travel throughout southern New Hampshire.

Duties & Responsibilities

• Perform comprehensive physical therapy evaluations in patients' homes.
• Develop, implement, and modify individualized treatment plans.
• Reassess patient progress and update care plans as needed.
• Coordinate care with physicians, nurses, caregivers, and other healthcare professionals.
• Educate patients and families on treatment plans, exercises, mobility, and safety.
• Document evaluations, treatments, and patient progress accurately in the electronic medical record.
• Supervise Physical Therapist Assistants (PTAs) in accordance with New Hampshire regulations and agency policies.
• Maintain compliance with organizational policies, safety protocols, infection control standards, and ethical guidelines.
• Participate in quality improvement initiatives and interdisciplinary team meetings.
• Travel throughout assigned service areas to provide home health physical therapy services.
• Participate in a rotating weekend schedule.

Requirements

• Graduate of an accredited Physical Therapy (APTA) program .
• Current New Hampshire Physical Therapist license .
• Current BLS certification .
• Valid driver's license , reliable transportation, and proof of auto insurance.
• Minimum 1 year of physical therapy experience .
• Basic computer and electronic documentation skills.
• Ability to travel throughout southern New Hampshire and participate in a rotating weekend schedule.
