Registered Nurse – Home Health

Schedule

8 hrs shifts, with potential weekend rotation as needed.

Assignment Details

Guaranteed Hours: 40 Contract Weeks: 15

Requirements

• Current state license as a Registered Nurse - Required
• Homecare Homebase Software Experience - Required
• OASIS Experience - Required
• 1-2 years experience as a Registered Nurse in a clinical care setting - Required
• 2+ years Home health experience - Required
• CHPN certification - Preferred
• Proof of current CPR certification - Required
• Current driver's license - Required
• Must have reliable transportation and insurance - Required
• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required
• Bachelors of Science in Nursing - Preferred

Responsibilities

• Assess home health and/or hospice patients to identify their needs.
• Implement, develop, document, and evaluate the Plan of Care to ensure continuity of care.
• Educate patients and families regarding care, medications, and home safety.
• Supervise and provide clinical direction to home health aides.
• Monitor assigned cases for compliance with payer requirements.
• Provide effective communication among patients, families, and team members.
